Application for Course Reimbursement Scholarship Program

Criteria for applicants:
1.      
Must be employed by the Cape Henlopen School District for the Sussex Consortium while taking the courses.
2.      
Paraprofessional staff: Funds available immediately.
3.      
Professional staff: District reimbursement funds must be exhausted prior to application with LDAF. Please submit your
      reimbursement refusal notice from the Cape Henlopen District Office along with documentation of course costs.

4.      
Professionals: Reimbursements considered for Autism Certification courses only.
      Paraprofessionals:
Reimbursements considered for Special Education courses only.
5.      
Money will be paid directly to the institution in the individual’s name, unless otherwise agreed upon between applicant
      and LDAF.

6.      
Must submit a description of course costs (bill or receipt) with this application. Course cost documentation must be
      submitted to LDAF by mail, fax, or scan/email. Requests will be considered by the LDAF Scholarship Committee upon
      receipt of your application and requested documentation.

7.      
Applicant may apply for tuition reimbursement for one course per semester up to $800.00.
8.      
Must complete the course with an A or B grade prior to making subsequent requests to LDAF.
9.      
Must volunteer at one LDAF function per grant awarded prior to any future requests for reimbursement.
10.  
Maximum of two reimbursements per year per person.
